Duties:
– Provide direct support and assistance to at-risk youth and young adults
– Help Implement individualized service plans with assistance from Case Management
– Teach life skills, such as job skills, housing plans, and educational achievements work toward self-sufficiency.
– Assist individuals in participating in community activities and social outings
– Collaborate with a case management team to ensure the overall success of the individual
– Document and maintain accurate records of progress and access data
Skills:
- A Human Services Degree is required.
- Experience working with at-risk youth and young adults with a diverse population
- Knowledge of the foster care system, homeless youth, and disconnected youth is a plus.
- Experience in data collection and documentation
